Informed Consent Form

This is the form that needs to be read and signed before your first counseling session.

Confidential Intake Form

This will include personal information, such as your reason for seeking counseling and any counseling history you may have. This form, once completed, will only be read by your counselor.

Reduced Fee Application

If you ever feel the need for a reduced fee for your sessions at FCC, we are ready and willing to help. Please fill out this application and give it to the Front Desk.
